A 38-year-old female presented with a recurrent left submandibular swelling that had been persisting for approximately two weeks.

History: The patient had a history of a prior surgical intervention for a sublingual mass, performed intraorally without histopathological examination. Following the surgery, the swelling recurred, leading to three drainage procedures before the patient sought consultation from the Smart Surgical Team.

Pre-operative CT scan revealed a well-defined cystic lesion measuring 31 × 25 × 21 mm in the floor of the mouth, extending into the left submandibular space. An ultrasound examination at our clinic showed a large, thin-walled cystic lesion, located anterior-inferior to the left submandibular gland, extending into the left sublingual region, measuring 36 × 25 × 6 mm. These findings raised suspicion of a complicated or infected Plunging Ranula.

The Multidisciplinary Team (MDT) recommended surgical intervention for the left submandibular and sublingual lesions.

After excision of the left submandibular and sublingual glands, the histopathological examination confirmed the diagnosis of retention mucous cyst (Ranula) with nonspecific sialadenitis, ruling out malignancy
