Case Presentation:
A 38-year-old female presented with a recurrent left submandibular swelling that had been persisting for approximately two weeks.

History:
The patient had a history of a prior surgical intervention for a sublingual mass, performed intraorally without histopathological examination. Following the surgery, the swelling recurred, leading to three drainage procedures before the patient sought consultation from the Smart Surgical Team.

Investigations:
Pre-operative CT scan revealed a well-defined cystic lesion measuring 31 × 25 × 21 mm in the floor of the mouth, extending into the left submandibular space. An ultrasound examination at our clinic showed a large, thin-walled cystic lesion, located anterior-inferior to the left submandibular gland, extending into the left sublingual region, measuring 36 × 25 × 6 mm. These findings raised suspicion of a complicated or infected Plunging Ranula.

Surgical Management:
The Multidisciplinary Team (MDT) recommended surgical intervention for the left submandibular and sublingual lesions.

Histopathological Examination:
After excision of the left submandibular and sublingual glands, the histopathological examination confirmed the diagnosis of retention mucous cyst (Ranula) with nonspecific sialadenitis, ruling out malignancy
